My sitemap.xml shows a modified date 2 years old. How is it created?
It's handled by sitemap2.aspx, you can see this file in the root of the site. But you need to have a source code to see how it's really created and go to ASPDNSFCore\AsSiteMapComponentArt.cs file.
Does ASPDOTNETSF create and update the sitemap.xml automatically, or do I need to run the process?
You have to actually submit this url to google https://www.yoursite.com/GoogleIndex.aspx, just follow this manual.
OK, I've done that. Google.com/Webmasters/Tools shows us there are x URLs but < x were indexed and when you view those urls, there are exact duplicates. Why would aspdnsf generate dups?
I can creat sitemap.aspx through the site of course, but how do I create sitemap.xml and googleindex.xml? Google DOES NOT accept aspx, ONLY xml.

Download gsitecrawler it's old but does the job and creates an xml file for your site that can be uploaded.
Be warned though it can take several hours to crawl a site with a few thousand products.
If you have PHP installed I would go down the XML-sitemap route. We currently use it and could not be any happier. We can set it automatically or manually, we like to do it manually for when products have just been added. Lots of control and very easy to use. Takes under an hour for just over a thousand products.
